After completed the final mission when I go into fantasy room on the small map i have bunch of lips icons but there are no girls at those locations, what can I do to fix this issue.

Are there any girls on the main floor or is that for expansion.
There's girls in the groto. They are found on the first floor. They are the ONLY girls at the ground level. All others are "upstairs".

There's nothing in the cave behind the stairs. Must I beat the game on hard before something appears?

Found them in the cave in the pool in the middle of cave, can not see them on main floor.
Yep. 2 girls making out and a single girl at in the center pool of the grotto.
